please give reasons to each
mine are:
cheogh(blood)
though the other bosses suck this guy owns! i see ophelias corpse on that wall and im like(oh... that sucks ;c) then caleb calls out in anger and the BOOM! OH SHIT! BIG! i litterally LEAPED from my seat! the bastards metric ton of health made him good too
cerberus(blood)
being insta slaughtered hasnt been this epic since,,, ever
xemnas(kingdom hearts II) huge deadly and awesome. its such a long time ago... but nonetheless i remibmer loving it more the the first final KH boss. the obvious anticipation that lets you decide was also nice(hmmm... ill go up grade my self a bit more before doing the end battle k?)
music
blood(again...)
the music allways fit and added and air of creepiness i disnt think build could pull of.
heretic (wow...)

Music:
First off, let me say that anything composed on an FM synthesizer makes me hard. I absolutely *LOVE* how they sound. Personal favorites are the Yamaha OPN2+TI PSG in the Sega Genesis and especially, ESPECIALLY The Yamaha OPL3, of Sound Blaster 16 fame (And every other knockoff sound card of the era.) I would go so far as to say that the OPL3 is my all time favorite instrument.
My favorite soundtrack of all time? Very difficult. Want to say Jet Grind Radio but I can't because it is 0% original.
Favorite would probobly be Descent 1. Notable entries with some really great tracks are Halloween Harry, Sonic CD, Sonic 3 and Knuckles (Go MJ!), pretty much anything Bobby Prince has touched (Doom, Major Stryker, etc.) too many to list.

Hmmm...
Bosses would have to go to Ultimecia and Griever from Final Fantasy 8. Absolute bitch to kill(canny do it without Holywars and preventing leveling up) and the four fights were perhaps the most spectacular of any RPG game I've ever played. Also, I completed FF8 late one night and was fucking mesmerised by it. Went to sleep and was woken up an hour later to be told my grandmother had been rushed to hospital. So FF8 is very close to me, so to speak. I bought two FF8 statues(Squal & Bahamut, Siefer and Odin) but never got to own a Ultimecia & Griever one...that would have been fucking cool indeed!
Story...dunno. Most probably FF8 again. Loved the romance parallel between Squal & Rinoa and Ultimecia & Griever. S&R was obviously a lovey-dovey kind of relationship, whereas U&G were like master and slave - like a mistress with her faithful and beloved dog. I always had this image in my head of a little chibi Ultimecia cuddling Griever as a little Kitten... ^_^ But yes, theres always been the idea/hint that S&R were U&G in the future, so thats a nice thought to play around with, whether it was the intention of the FF8 team or not. I wished they made a Statue of Ultimecia to coincide with the release of Dissidia...instead all we get a poncy figure of that twat from FFX...
Music. Bloody hell thats a hard one! I've always shined to most of the BGMs of Dragon Spirit(Namco, 1987). The first level is absolutely legendary, whilst for me the best is when the lights go out in Zawel's castle and come back to reveal the three-headed hydra guardian. Its really clever to be honest, because it suggests that the princess is nearby, and to fail then would be tragic, and that it gives a feeling of taking on a titan that maybe invincible. Contrasted with the more heroic and "thunderous" theme of Zawel's battle, its a very good prelude to that said fight. Whoever did the soundtrack for DS should be given the victoria cross for sheer awesomeness.

I can never set a final top three, but here are some that are definitely worthy:
Bosses:
- The sniper battle with "The End" from Metal Gear Solid 3: Snake Eater
- M. Bison on maximum difficulty (level 7, I think) from Street Fighter II on SNES
- Lavos, final battle from Chrono Trigger
Music:
- Chrono Trigger
- FInal Fantasy 7 and 8
- Gran Turismo 1
Story:
- Chrono Trigger
- Earthbound / Final Fantasy 7 franchise Tie
- Metal Gear Solid Series
There are so many more games that need to be on a top three list, you can't really call it a top 3 anymore, but these are some good examples. Top 50 or Top 100 might be more appropriate.

I see a lot of console answers here. Im not much of a console gamer so most of my answers will be pc oriented.
Bosses:
- Goro (mortal kombat)
What can i say, he was fucking badass (Shang Tsung was a pussy). And the stop motion feel he had just gave him that extra creepy & thoughness factor.
- The purple tentacle (day of the tentacle)
The whole story behind the pruple tentacle was just simple genious. After comming in contact with toxic waste he grew some 'arms' and wants to dominate the world.
The way he was animated and voiced was just brilliant also.
- Shin Akuma (Street fighter 2: Turbo)
I love how he was integrated as an alternative to mr Bison if you were good enough.
When you got to Bison quite fast, he appears, kicks Bison's ass and you get to fight this guy instead.
Music:
- Motorhead
The music fitted this game verry well and was of great quality.
Verry dreamy and relaxing. Just sit back and enjoy the ride.
The fact you could just pop the disc in any cd player and play the music is defenetly a plus.
- Outcast
Some really nice music here performed by the Mosco music orchestra that really intensifies the experience of this great game.
And also here, you can just play the disc in a standard cd player. Awesome.
- Tetris
Just for fun, haha. Those tracks drive you totally insane. lol
Story:
- Outcast
Just awesome how well this game was put togetter.
- Baldur's gate
Best RPG on pc ever! The story was well put togetter and the interaction between the characters was awesome.
There were so many plots and subplots... Its one of the few games that really made me think about possible consiquences later on before i made some choices.
I probably forgot some, but well...
